Ito ay gaganapin sa San Pedro Astrodome, Brgy. Pacita 1 (8:00 AM – 5:00 PM) at magtatapos hanggang sa February 21, 2024 (Biyernes).
Narito ang mga kinakailangang dokumento:
A. ON TRICYCLES FOR HIRE
1. New Applicant/s
a. Barangay Clearance (specifying the allowable garage)
b. Photocopy of latest Certificate of Registration and Official Receipt (C.R./OR) of motorcycle/tricycle from the Land Transportation Office
c. Certificate of Membership from an existing TODA in the locality where the tricycle is to operate to ensure smooth operation and harmonious relationship with existing members
d. Photocopy of Deed of Sale or Deed of Conveyance/Transfer
e. Photocopy of Insurance Coverage for third party liability
f. 2 x 2 I.D. picture (wearing TODA uniform)
g. Photocopy of Professional Driver’s License
h. TODA Clearance
i. TODA Uniform
j. TODA Roof Color Coding
k. Trash cans
l. Roof number tagging
NOTE: All photocopy requirements submitted must also present their original copies for verification purposes.
2. Renewal of Franchise
a. Barangay Clearance on Garage (specifying the allowable garage) with attached pictures
b. Latest Certificate of Registration and Official Receipt (C.R./O.R.) of motorized tricycle from the Land Transportation Office (LTO) certifying that the unit is completely classified as “FOR HIRE”
c. Copy of previous Franchise/MTOP
d. Insurance coverage for third party liability
e. 2 x 2 I.D. picture (wearing TODA uniform)
f. Professional Driver’s License
g. TODA Uniform
h. TODA Roof Color Coding
i. Fair Matrix Guide
j. Trash Can
k. Roof number tagging
B. FRANCHISING REQUIREMENTS FOR SERVICE/PRIVATE MOTORIZED TRICYCLE
a. Barangay Clearance on Garage (specifying the allowable garage)
b. Certificate of Registration (C.R.) and Official Receipt (O.R.)
c. Annual Safety Inspection of motor vehicle
d. Insurance of motor vehicle
e. 2 x 2 I.D. picture (2 pcs.)
f. Previous Franchise
g. School Permit (for school service)
h. TODA Clearance
i. Professional Driver’s License
j. I.D. with specific time and route of service
k. Color Code (roof should be painted with Yellow and Black stipes)
l. Trash can
Note: Business Establishments using tricycles for delivery of goods are required to apply for service Franchise.
