๐๐ฎ๐๐ฐ๐ต ๐ฎ๐ฌ๐ฎ๐ฎ – (June 9, 2023 2:00pm to 5:00pm)
Please bring the following to the City Treasury Office:
โข ILSP Claim Stub
โข Original & Photocopy of school ID with three “3” specimen signatures
For authorized persons who will be claiming on behalf of the scholar, please bring the following:
โข ILSP Claim Stub
โข Original Authorization letter duly signed by the scholar.
โข Original & Photocopy of authorized person’s ID with three “3” specimen signatures
โข Original & Photocopy of school ID with three “3” specimen signatures
