– must be a licensed Medical Technologist

Physical Therapists
– must be a licensed Physical Therapist
– with or without prior work experience
Occupational Therapists
– must be a licensed Occupational Therapist
– with or without prior work experience
Pharmacists
-must be a licensed Pharmacist
-with or without prior work experience
Farm Worker
– must have related work experience
Plumbers
– must have related work experience
Clearing Operative
– must be at least an elementary graduate
– with or without prior work experience
For walk-in applicants, kindly prepare and submit the following:
• Two (2) fully Accomplished Personal Data Sheet
• Two (2) Passport Size Picture
*(3.5×4.5cm) with handwritten Name and Signature
*White background with collar and no facial hair
