  1. Fully accomplished Personal Data Sheet (PDS) (3 copies) with recent passport sized picture (CS FORM No. 212, Revised 2017) which can be downloaded  here. Please make sure the you read the guide in fill up the Personal data Sheet (PDS).
  2. Photocopy of certificate of eligibility/rating/license
  3. Original and photocopy of transcript of records
  4. NBI Clearance
  5. NSO Birth Certificate
  6. NSO Marriage Certificate (for married female)
  7. IPCR for Two (2) consecutive period (for promotions)
